Letter of Medical Necessity

The Wellness Collective is a physician-created wellness education program that may qualify for HSA/FSA reimbursement.

Download the  Wellness Collective Letter of Medical Necessity.docx.  Bring the letter to your physician and have him/her sign it.  A single Letter of Medical Necessity can cover 6-12 months of membership, so your doctor doesn't need to sign anything monthly.  Just save your receipts and submit them together with your Letter of Medical Necessity to your HSA/FSA provider.

Not guaranteed, but definitely worth a try. Simplified wellness + savings = a great combo.
